Healing Starts With a Home
Individuals with chronic health conditions are at far greater risk of experiencing homelessness than the general population. These conditions can become disabling, making it difficult for the individual to maintain stable relationships, employment, and housing on their own. At Columbus House, our case management services connect people dealing with health issues to the critical health, recovery, and housing resources they need to overcome homelessness.

Adriana had a great education, was a business professional, and raised an independent daughter, but she says, “I discovered late in life that I was not immune to the perils of substance abuse.” After several attempts to overcome her substance use disorder, Adriana lost everything – including her home.
Rapid Re-Housing
Rapid Re-Housing is a program for people with a qualifying disability that moves them directly from instances of homelessness into their own rental unit by providing short-term financial assistance in the form of initial rent, security deposits, and/or utility assistance. Each year, we help over 150 people get into their own homes in New Haven and Middlesex counties through this program.
Recovery House
A short-term program for 17 men (including 7 Veterans) who are awaiting placement in sober housing, treatment programs, or permanent housing with outpatient treatment support.
Sojourners Place
A New Haven supportive housing program serving people who were experiencing homelessness and are dually-diagnosed, i.e., have a history of both mental illness and substance abuse.
Road to Recovery
This transportation program supports eligible individuals enrolled in HUSKY D or the Military Support Program by helping them get to and from recovery-related destinations, including sober homes, recovery houses, shelters, detox and treatment centers, hospitals, VA medical centers, and Vet Centers. To qualify, individuals must be receiving HUSKY D or MSP services and actively connected to treatment.
Medical Respite Program
This innovative program provides recuperative care to those who are homeless and exiting the hospital. In addition, the program lowers public healthcare costs by reducing unnecessary hospitalizations. Based out of our New Haven Shelter, referrals come directly from Yale New-Haven Hospital and the VA Hospital in West Haven. The 12-bed program includes 24-hour supervision, referrals to healthcare providers, transportation, and case management.
